How much room your diaphragm has to move depends heavily on sleep posture — and most people are compressing it every night without realizing it.

The posture swap: side sleeping, spine neutral

Side sleeping with a relatively straight spine gives the diaphragm the most room to expand the ribcage and abdomen on each breath, compared to back sleeping (which can let the tongue and soft palate partially restrict the airway) or a tightly curled position (which compresses the ribcage directly). If you're currently a back or stomach sleeper, this is the single highest-leverage change available.

Pillow height matters more than pillow softness

A pillow that's too high tilts the neck forward and can partially narrow the airway even in a good side-sleeping position; too flat, and the spine curves the other way. The target is a pillow that keeps your neck roughly level with your spine when lying on your side — neither craned up nor collapsed down. This is a fit problem more than a firmness problem, and it's worth adjusting even if the pillow itself feels comfortable.

A pillow between the knees, if you're a side sleeper

For side sleepers specifically, a pillow between the knees keeps the hips and lower spine aligned, which indirectly keeps the ribcage and abdomen from twisting out of a neutral position over the course of the night. It's a small addition, but it helps the good posture you started with actually hold for eight hours instead of drifting.

One change, measured over time

You won't feel this shift directly — nobody consciously notices diaphragm compression while asleep. But if you track sleep quality with a wearable, this is one of the few single changes worth testing in isolation for a week or two, since it targets a mechanical constraint rather than a behavioral one, and mechanical constraints tend to respond quickly and consistently once removed.
