Your vestibular system, housed in the inner ear, senses motion and balance. Your eyes report visual motion. Normally these two systems agree with each other. In a moving car while reading, your inner ear senses the car's motion (acceleration, turns, bumps), but your eyes, focused on a stationary page, report that you're not moving at all. That mismatch — one system says motion, the other says stillness — is the root trigger of motion sickness.
Why the brain jumps to "poison"
The leading explanation, called sensory conflict theory, proposes that this specific kind of mismatch historically had one common real-world cause: certain neurotoxins and poisons disrupt the nervous system in ways that produce exactly this sensory disagreement. Without a way to distinguish "car ride" from "neurotoxin exposure," the brain defaults to the interpretation it evolved around, and triggers nausea and vomiting as a protective response — the body's way of trying to purge whatever it assumes was ingested.
Why it hits some people harder than others
Susceptibility varies a lot, and is generally higher in children (whose sensory-integration systems are still developing) and tends to decrease with age. Motion sickness is also strongly tied to context — the mismatch is much stronger when actively reading or looking at a phone than when watching the road ahead, because looking at the horizon gives the visual system input that actually matches the vestibular system's motion signal.
